Herb-Drug Interactions

2024-12-20 - 2024-12

The possibilities of drug interactions, direct toxicities, and contamination with active pharmaceutical agents are among the safety concerns about dietary and herbal supplements. Although there is a widespread public perception that herbs and botanical products in dietary supplements are safe, research has demonstrated that these products carry the same dangers as other pharmacologically active compounds. Interactions may occur between prescription drugs, over-the-counter drugs, dietary supplements, and even small molecules in food—making it a daunting challenge to identify all interactions that are of clinical concern.
Concerns about herb-drug interactions are often not based on rigorous research. Most herb-drug interactions identified in current sources are hypothetical, inferred from animal studies or cellular assays, or based on other indirect means; however, attention to this issue is needed for drugs with a narrow therapeutic index, such as cancer chemotherapeutic agents, warfarin, and digoxin.
To date, well-designed clinical studies evaluating herbal supplement–drug interactions are limited and sometimes inconclusive. This issue of the Digest provides information about several herbs and their potential interactions with other agents.

While normally beneficial or harmless, natural supplements and medicines can be harmful if used in excess

2024-12-24 - 2024-12

Few people would pop a medical pill without a doctor’s prescription or advice. The caution about self-medication or overuse, however, tends to drop when the pill in question is herbal. The common assumption is that since herbal pills are derived from natural sources, they must be safe to use freely without restrictions on courses and dosages.
This is a dangerous misconception.
The rise in use of herbal medicines and dietary supplements (HDS) over the years has increased the incidence of liver injury and damage, with people sometimes requiring liver transplants or even dying. In the US alone, HDS-related liver failure has increased eightfold over the past 25 years. A study from Korea found a similar trend, reporting that HDS overuse is linked to drug-induced liver injury which involves severe liver enzyme abnormalities, liver damage, the need for liver transplants and even death.

An unregulated market
In 2022, the global herbal medicine and supplements market was valued at US$70.1 billion, with projections to grow to US$389.75 billion by 2031. The European Union is the largest consumer of HDS, accounting for 50 per cent of global consumption.
With their increasing usage, drug-induced liver injury is becoming a growing health concern globally.
During the COVID-19 pandemic, many consumers turned to HDS products to boost their immune systems, using them as both preventive and therapeutic treatments. In the US, HDS sales surpassed US$10 billion in 2020.
Part of the recent hype surrounding HDS is because the US Food and Drug Administration classifies them as food supplements. Unlike prescription drugs, HDS are not subject to regulation because they are plant-based and can be sold over the counter.
The use of such herbal medicines is part of ancient healing traditions in many cultures and countries. In India, China, Korea and Singapore, among other places, traditional herbal medicines are officially recognised and integrated into public healthcare systems.
The health benefits of several such medicines and supplements are well-established.

Predictive, Preventive and Personalized Medicine: Leads From Ayurvedic Concept of Prakriti (Human Constitution)

2024-12-25 - 2024-12

Purpose of Review Genomics has ushered in a phase of predictive, preventive and personalized medicine (PM), and several attempts are being made to detect genetic variations that are responsible for susceptibility to diseases and varied response to medications, however, have met with only a limited success. Ancient Indian system of medicine, Ayurveda, strongly emphasize on personalized patient care under the concept of Prakriti (phenotype-based human constitution). We would therefore like to put forward a new dimension of PM—the ‘Prakriti-based medicine’ bearing testable molecular and genetic correlates. Recent Findings Current investigations in the field of genomics, PM and Ayurveda have inspired many researchers and started in-depth deliberation on how Prakriti (phenotype-based Ayurveda constitution) is associated with the field of genomics, biochemistry, psychology, physiology and therapeutics. Various genotypes have also been correlated with Prakriti. Studies have shown wide range of utilities of Prakriti assessment in therapeutics, i.e. from predicting disease susceptibility of an individual, prevention of impending diseases, early diagnosis through screening of the high risk, rational drug designing, customization of therapy (drug, diet and lifestyle) and health maintenance. Summary Ayurvedic Prakriti-based treatment resonates with PM and pharmacogenomics and holds potential and promise for future predictive or preventive medicine. Further studies are warranted towards integrating or complementing genomics and contemporary medical science with Prakriti and explore the future possibilities.

Gujarat: Meeting to discuss ‘practice of Allopathy by those who have studied Ayurveda’ sparks row

2024-12-31 - 2024-12

On Monday, the IMA-GSB wrote to Gujarat Health Commissioner Harshad Kumar R Patel, drawing attention to what it called the “critical issue of unauthorised practice of modern medicine by non-allopathic practitioners and quacks.”
A letter from Gujarat health department calling a a meeting of officials and regulators to discuss “the practice of allopathy by those who have studied Ayurveda” has not gone down well with the Gujarat State Branch of the Indian Medical Association (IMA-GSB), which has stated in a letter that allowing “this dangerous trend” could lead to medical emergencies and “jeopardize the public’s trust in the medical system.”
The IMA-GSB members have also taken exception to not being invited to the meeting scheduled for January 3 in Gandhinagar. This comes two days after the Food and Drugs Administration (FDA) in Maharashtra permitted prescription of Allopathic medicines by Homeopaths who have completed a course in modern pharmacology.
On Monday, the IMA-GSB wrote to Gujarat Health Commissioner Harshad Kumar R Patel, drawing attention to what it called the “critical issue of unauthorised practice of modern medicine by non-allopathic practitioners and quacks.”
The IMA-GSB letter stated several instances of individuals providing Allopathic treatments without a medical education in modern medicine had surfaced and that “this trend posed a serious threat to the health of the people, leading to dangerous and sometimes life-threatening consequences for unsuspecting patients.”
The scheduled meeting, to be held under the aegis of Dhananjay Dwivedi, the Principal Secretary of Health, came as a result of a representation by one Kamlesh C Rajgor, who runs a private hospital in Ahmedabad.
The health department has sought attendance from Commissioner of Health, MD of National Health Mission , Additional Director of Medical Services, Additional Director of Medical Education and Research, President of the Gujarat Medical Council, President of the Gujarat Board of Ayurvedic and Unani System of Medicine (GBAU), President of the Council of Homeopathy System of Medicine, and Dr Rajgor, who runs Yogeshwar Multispeciality Hospital.
The agenda of the meeting, the letter states, is to discuss “whether those who have become doctors in the study of Ayurveda can study Allopathy if the need arises.” When contacted, Dr Rajgor declined to comment on the contents of his representation.
Dr Sanjay Prabhudas Jivrajani, the president of the GBAU, said: “The term ‘quackery’ is unfortunately extrapolated to include Homeopathy and Ayurvedic practitioners. We want to help correct this narrative. We also want to make sure that if doctors under our Board are crossing any limits, they must learn to remain within their mandate.” On IMA’s letter, Dr Jivrajani said that all systems of medicine are “three pillars of the Indian health system and must work together.”

2 pretending to be Ayurvedic medicine sellers loot gold, cash from woman’s house
RAJKOT: Two people pretending to be Ayurvedic medicine sellers robbed gold and cash from the house of a 58-year-old woman after tying her hands and legs in Jamnagar city.
Following vital clues from CCTV footage and using human intelligence, the police apprehended both the accused from Porbandar, a few hours after the incident that occurred in the house of Farida Ataria in Tamaramad Society on Monday afternoon.
The accused were identified as 28-year-old Hitesh Kharvani and 21-year-old Dharmik Kharvani, and the police are verifying if they have a prior criminal record. Police also recovered 240 grams of gold, including a chain and biscuits, and Rs one lakh cash from them.
The incident unfolded when the duo approached Ataria's house, pretending to be sellers of Ayurvedic medicines. Barely did Ataria initiate a conversation when they pushed her to the floor, assaulted her, gagged her mouth with cloth, and tied her hands and legs with rope. They even attacked her son and then carried out the robbery.
Jamnagar superintendent of police (SP) Premsukh Delu said the cops arrested the two accused within 5-6 hours of the incident. "Following the complaint, the police immediately cordoned off the area and inspected the crime scene. CCTV footage from nearby locations was reviewed, and three special teams were formed to investigate the case," Delu said.
To track down the accused, the police reviewed CCTV footage from the Command and Control Centre, which revealed that the duo used a motorcycle to flee to Porbandar. Both are natives of Porbandar town.
The police are now probing why Ataria and her family were specifically targeted and whether the robbers acted on a tip-off from someone else.
The Jamnagar City police booked the duo under relevant sections of BNS.

Babool: 5 Lesser Known Health Benefits Of This Medicinal Plant

2025-01-03 - 2025-01

Babool, also known as Acacia nilotica, is a tree revered for its medicinal properties in traditional and herbal medicine systems. Widely found in India, Africa, and the Middle East, this plant has been used for centuries in Ayurveda and other natural healing systems for its myriad benefits. Let’s explore five scientifically-backed health benefits of babool and how it contributes to overall well-being.
Babool is a natural remedy for dental health, with its bark and gum being widely used in toothpaste and mouthwashes. Its antibacterial and astringent properties help prevent tooth decay, fight gum infections, and reduce bad breath.
A study published in the Journal of Ethnopharmacology (2015) found that Acacia nilotica extracts significantly reduced plaque formation and bacterial growth, making it an effective agent for maintaining oral hygiene. The bark of the babool tree contains tannins, which strengthen gums and help in the prevention of periodontal diseases.
How to Use: Chewing on babool twigs (datun) is a traditional practice in India to clean teeth and maintain oral health.
2. Supports Digestive Health
Babool is a powerful remedy for managing gastrointestinal issues. Its high fiber content aids digestion, while its antimicrobial properties help combat harmful bacteria in the gut. Babool gum is also used to manage diarrhea and dysentery effectively.
A 2018 study published in the Journal of Medicinal Plants Research highlighted the role of babool in reducing inflammation in the gastrointestinal tract. The flavonoids and polyphenols in babool possess protective properties that soothe the stomach lining and improve digestion.
How to Use: Babool powder can be mixed with water or honey and consumed to alleviate digestive discomfort.
3. Heals Wounds and Skin Conditions
The antibacterial and anti-inflammatory properties of babool make it a natural choice for treating wounds and skin infections. The leaves and bark extracts are commonly used to speed up wound healing, reduce swelling, and prevent infections.
A study in the Indian Journal of Pharmaceutical Sciences (2020) found that Acacia nilotica exhibited significant wound-healing activity due to its ability to promote collagen synthesis and tissue regeneration. Unveiling the Mask: Gender Dysphoria and the Path to Holistic Healing

2025-01-01 - 2025-01

Gender dysphoria and gender incongruence occur when an individual’s gender identity differs from their biological sex, often resulting in significant distress. This report examines a 34-year-old male patient with severe obesity, depression, metabolic syndrome, and cardiovascular disease, who visited his family doctor in June 2021 following a recent diabetes diagnosis. Notably, the patient was newly recognized as experiencing gender dysphoria, which is believed to have been present for many years. This previously unrecognized dysphoria may have contributed to various comorbidities throughout his life, including social isolation, depression, obesity, and cardiovascular disease. The case highlights the importance of a holistic healthcare approach, emphasizing the integration of social context, mental health, and gender identity with physical health considerations. This comprehensive perspective enabled the physician to identify a potential underlying factor contributing to the patient’s medical challenges. The case also illustrates the interconnectedness of mental and physical health and underscores the value of a strong doctor-patient relationship, which created a safe space for the patient to disclose his long-held gender dysphoria for the first time.
Introduction
The current criteria for diagnosing gender incongruence involve persistent incongruence between an individual’s gender identity and their sex assigned at birth, which is typically determined based on external sexual anatomy. This diagnosis is essential for considering gender-affirming hormones and surgical therapies. Additionally, individuals may be diagnosed with gender dysphoria, defined as the distress stemming from this incongruence rather than the incongruence itself. Diagnostic criteria are primarily guided by the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5), which emphasizes persistent discomfort with the mismatch between gender identity and external anatomy, as well as significant interference in areas such as social or academic functioning [1,2].
0.5% of the adult population identify as transgender [3]. However, precise global figures remain elusive, complicating healthcare planning. Transgender individuals represent a diverse population, encompassing those who live with gender incongruence without transitioning, those who transition socially without seeking medical intervention, and many who face barriers to accessing care due to stigma or reliance on nonmedical resources. Consequently, research often focuses on individuals accessing gender-affirming care at specialized clinics, thereby underestimating the broader transgender population and excluding those who might benefit from nonclinical resources like information and counseling. A recent study extrapolated the 0.5% prevalence rate to the global population aged 15 years and older, estimating approximately 25 million transgender individuals worldwide [4].
In Portugal, accurate data on the p

To promote a balance between mental and physical health, International Mind and Body Wellness Day is celebrated on January 3rd every year, a global initiative first observed in 2019.
This day reminds us that health isn’t just about physical fitness—it’s about mental wellness too.
Psychosomatic Diseases and Mental Health
Many health problems today are classified as psychosomatic diseases, which are triggered by poor lifestyle choices, unresolved emotions, or stress. The increasing pressure on individuals, whether at work, in relationships, or in social settings, has led to an alarming rise in mental health issues.
To combat this, International Mind and Body Wellness Day emphasizes adopting strategies that improve mental and physical well-being, like yoga, meditation, and self-care practices. It’s a reminder to take proactive steps toward a healthier and more balanced life.
Understanding the Mind-Body Connection
Philosophers and spiritual leaders have long discussed the strong link between the mind and body. Ancient approaches, such as Ayurveda in India and traditional Chinese medicine, advocate for holistic health by treating both mental and physical aspects of well-being.
Engaging in physical activities while also focusing on mental health practices like spending time with loved ones, reading motivational books, and practicing meditation can lead to better overall health. These habits can reduce the risk of chronic diseases, promote sound sleep, and help maintain a balanced lifestyle.
Famous Quotes on Wellness
Several renowned personalities have spoken about the importance of mental and physical health. Here are a few motivational quotes:
1. “People are just as happy as they make up their minds to be.” – Abraham Lincoln
2. “If you want to live a happy life, tie it to a goal, not to people or things.” – Albert Einstein
3. “Happiness is the highest form of health.” – Dalai Lama
4. “A fit body, a calm mind, a house full of love. These things cannot be bought; they must be earned.” – Naval Ravikant
5. “Self-care is not selfish. You cannot serve from an empty vessel.” – Eleanor Brown
Five Best Ways to Attain Holistic Health
Here are five key strategies to achieve holistic health for a better balance between mind and body:
1. Do a SWOT Analysis: Assess your Strengths, Weaknesses, Opportunities, and Threats. This can help you turn challenges into opportunities and manage stress effectively.
2. Reflect and Reconnect: Take time to reflect on positive solutions to your problems. This will help you regain control over stressful situations.
3. Engage in Your Favorite Activities: When things don’t go as planned, shift your focus to activities you enjoy. Whether it’s composing a poem, singing, or doing charity work, these activities boost happiness.
4. Adopt a Healthy Life

Planetary Orbital Surveys Using A Tetracorder

2025-01-06 - 2025-01

In Star Trek, the intrepid explorers use a handheld device called a tricorder to sense, record and compute environmental data. But scientists in the real world have one-upped our fictional friends with the invention of Tetracorder.
Tetracorder takes imaging spectrometer data – or patterns of light emitted or reflected from an object – from huge swaths of land, sea and sky and quickly analyzes it. In a new paper, published in the Planetary Science Journal, researchers explain that Tetracorder analysis has taken another developmental leap that allows quick production of mineral maps. The paper also reports on field research indicating that the technology can be deployed on a rover to help guide autonomous exploration.
Planetary Science Institute Senior Scientist Roger Clark is lead author and PSI Senior Scientists Eldar Noe Dobrea and Amanda Hendrix and PSI Laboratory Technician Neil Pearson are co-authors.
A dozen researchers, led by Clark, have been working for decades to develop and refine this technology. It is currently being used by NASA’s Earth Surface Mineral Dust Source Investigation, or EMIT, instrument onboard the International Space Station to map minerals, dust, ice and snow, water and more on the Earth. EMIT was recently granted a mission extension.
“Back in the 1980s and 90s, when we were starting to get really good spectra of other planets, researchers would spend weeks, if not months, analyzing a handful of spectra,” Clark said. “Then imaging spectrometers came along. So instead of a few dozen spectra, we would be getting millions of spectra per minute. The whole game had to change in order to be able to analyze this amount of data.”
Tetracorder autogenerates maps of hundreds of compounds before synthesizing many into colored maps with multiple compounds denoted on each. These can include maps of water and impurities in the water, organic compounds and minerals and their grain sizes, for example.
“It is now easy to define what compounds one wants on a particular map for a specific science problem, and then have it auto-generated as new data arrives,” Clark said.
Tetracorder was recently used to find water and hydroxyl on the Moon. It will also be used to harvest rich information about Europa, Jupiter’s ocean moon encapsulated in ice, from data returned by the Mapping Imaging Spectrometer for Europa, or MISE, instrument onboard Europa Clipper, which launched Oct. 14, 2024.
“One major thing MISE and Tetracorder will help us determine is Europa’s surface composition,” Clark said. “This will allow help us determine Europa’s habitability. There’s evidence for acid that’s been frozen, such as sulfuric acid, but that would be hostile to life. We also expect salts to come up from an ocean below. A small amount of salt in the ocean would be hospitable to life, but too much salt might reduce habitability.”
The results from Clipper can also be used to target best locations for a future lander, and a rover w

Haryana: AYUSH dept mulls ways to ensure remunerative prices to herbal growers

2025-01-08 - 2025-01

Ayush director general Sanjeev Verma said due to the non-availability of any market and poor selling options, the farmers cultivating medicinal plants did not get fair prices
Haryana AYUSH department on Tuesday brought together farmers cultivating medicinal plants and manufacturers of ayurvedic medicines to explore the possibilities of providing remunerative prices to farmers. A meeting of the two was organised by the state medicinal plants board, AYUSH department.
Ayush director general Sanjeev Verma said due to the non-availability of any market and poor selling options, the farmers cultivating medicinal plants did not get fair prices.
“A total of 4,000 farmers are cultivating medicinal plants in Haryana. Hence, a meeting of pharmacy owners and medicinal plant cultivators was organised. About 30 farmers and 20 ayurvedic medicine manufacturers participated in this event,” he said.
A department spokesperson said farmers did not get proper markets and prices for the raw products they produce. The pharmacy owners suggested that cultivators should produce high-quality medicines by using proper drying facilities, storage facilities and primary processing of their raw material so that drug manufacturers can buy it directly.
It was also suggested by the ayurvedic medicine manufacturers to host an online portal on which farmers will be able to register and enter the details of the cultivated medicinal plants. The portal will help the pharmacy owners to purchase directly from farmers. The director general said there were various schemes offered by the central government, under which financial assistance can be arranged, and with its help, farmers can sell their produce at a higher price in the market and improve their economic condition.

The Hindu Lit for Life | Dr Issac Mathai on holistic health, integrative medicine, and the importance of research

2025-01-15 - 2025-01

In his career spanning decades, Dr Issac Mathai has seen patients from across the world, both at his facility SOUKYA, an international holistic health centre in Bengaluru, and as a visiting consultant in several holistic health centres in the UK, Europe and the United States.
It is this global perspective, and the importance of holistic healthcare and integrative medicine that he will be highlighting, at The Hindu Lit for Life 2025, in session with Dr E S Krishnamoorthy, neuropsychiatrist, author and the founder of Buddhi Clinic, and Vasudha Rai.
“The future of medicine is combining western and traditional systems of medicine. India is especially lucky to have all these systems and government policies that support it. It is important here to understand that the focus is on better healthcare, and not being against any particular system,” Dr Issac, founder, Chairman and Managing and Medical Director of SOUKYA says. With holistic health being a subject close to his heart, Dr Issac has authored two books — Dr Mathai’s Holistic Health Guide for Women, and Holistic Healing. “During the pandemic, I worked on my third book, Dr Mathai’s ABC to Health. Post the pandemic, people are much more aware of the importance of their immune system, and are eager to do things to improve their health,” he says.
While their facility in Bengaluru has seen a number of visitors from across the globe including King Charles III and Queen Camilla who were last there for a three-day rejuvenation retreat in October 2024, SOUKYA is also expanding internationally and has announced its first international centre in Portugal. Dr Issac has also been working on making complete health care accessible to all through holistic and integrative medicine through the SOUKYA foundation, and the Dr Mathai’s Rural Holistic Health Centre (DMRC) also provides healthcare free of cost to people in over 150 villages across the country.
Dr Issac acknowledges that there has been a lot more awareness and chatter about the importance of a healthy work-life balance. “People are beginning to think about how they can go about this, and it is important to take to lifestyle management in a better way. Hospitals or doctors including me cannot give you health and this is something people need to cultivate through a number of ways, be it through eating better, exercise, yoga and more. This is a vast area of holistic medicine and integrative medicine comes into the picture when you opt to use a tool beyond this,” he says.
While there are western medical institutions in India that are coming forward to delve into research in these alternate systems of medicine, Dr Issac says that scientific, modern research on a large scale is the need of the hour. “When you are dealing with systems of medicines that are thousands of years old, many of its practices and benefits have been in use for many years now. However, research is very much needed and India’s unique positioning with regard to these systems is some

Therapeutic potential and pharmacological mechanisms of Traditional Chinese Medicine in gout treatment

2025-01-17 - 2025-01

Gout is a systemic metabolic disorder caused by elevated uric acid (UA) levels, affecting over 1% of the population. The most common complication of gout is gouty arthritis (GA), characterized by swelling, pain or tenderness in peripheral joints or bursae, which can lead to the formation of tophi. At present, western medicines like colchicine, febuxostat and allopurinol are the primary treatment strategy to alleviate pain and prevent flare-ups in patients with GA, but they have significant side effects and increased mortality risks. Traditional Chinese medicine (TCM) has been utilized for thousands of years for the prevention and treatment of GA, demonstrating effective control over serum UA (SUA) levels with fewer side effects. Herein we summarized a total of 541 studies published from 2000 to 2023 in sources including PubMed, Web of Science, the Cochrane Library and Embase, highlighting the therapeutic potential of TCM in treating gout and GA, particularly in combination with modern medical strategies. This review focuses on TCM formulas, Chinese herbal extracts, and active compounds derived from TCM, providing an overview of recent clinical application and the pharmacological research based on animal models and cellular systems. Particularly, the current review categorized the clinical and experimental evidence into the strategies for improving hyperuricemia, decreasing the sudden onset of acute GA and retarding chronic GA progression, supplied further coherent reference and enlightenment for clinicians, investigators of natural product chemistry, researchers in TCM and pharmacology. We hope this article will inspire the development of novel formulas and molecular entities for the treatment of gout and GA.

Iran ranks third in traditional medicine trials in ICTRP

2025-01-19 - 2025-01

China and India rank first and second, respectively, the health ministry’s website reported.
The share of clinical trials in traditional and complementary medicine in Iran makes up about three percent of all clinical trials registered in the country, while just about 0.3 percent of the healthcare staff are experts in traditional and complementary medicine. The report shows the high capacity of Iranian researchers to move towards evidence-based medicine.
****Intl. Symposium on traditional, complementary medicine
The international symposium on the policy, legacy, and regulatory landscape of traditional and complementary medicine was held from November 21 to 23, 2024 in Tehran.
The symposium was held in person and online. By holding five specialized panels, the event served as a platform where experts from different Asian, European, American, and African countries discussed their views on traditional medicine.
Training and higher education in traditional and complementary medicine; integration of traditional and complementary medicine in the health care system; and laws and regulations in standardization and provision of traditional and natural products were among the main topics of the event.
The event provided a platform to boost communications, discuss, and share global experiences on traditional medicine laying the ground for integrating traditional and complementary medicine into the healthcare system, the health ministry website reported.
“It is essential to establish and implement policies, regulations, and legislations to be able to integrate traditional and complementary into the healthcare system,” the health ministry’s website quoted Arman Zargaran, an advisor to the traditional medicine office at the Ministry of Health, as saying.
The symposium can help figure out the best strategies to address challenges in integrating traditional medicine into the healthcare system, as well as ways to expand it, he further noted.
Health maintenance, disease prevention
Iranian traditional medicine strongly focuses on prioritizing health maintenance and disease prevention over treatment.
It is one of the most ancient forms of traditional medicine. It is grounded in the concept of four senses of humor: phlegm (Balgham), blood (Dam), yellow bile (Safra’), and black bile (Sauda’). The concept of the four senses of humor is based on the teachings of Rhazes and Avicenna in an elaborate medical system.
So far, about 30,000 plant species have been identified in the world, with Iran’s share of about 8,000 species which is more than the whole of species found in Europe.
Around 500 Persian medicine experts are providing health and medical treatment services across the country.
More than 1,000 general practitioners who have passed the Persian medicine courses approved by the health ministry are also offering services, Mehr news agency quoted Nafiseh Hosseini-Yekta, the director of the health ministry’s Persian medicine office, as saying.
